Can the Baby Book be exported to PDF?

One of the common questions from users of the NowServing Baby Book is the downloading of entries as PDF. While downloading to PDF is supported, you can only do so for the vaccine tracker entries. Growth trackers’ entries are downloadable to PNG File, while you can’t download the milestone trackers entries.

Most parents ask this question to convert this online baby book to a physical baby book. If that’s the plan, we recommend you to have a separate paper book instead of downloading and printing the entries from the NowServing Baby Book.

Can I add photos and writings in the digital baby book?

Another question we got from our users is if they can add photos and writings to personalize their baby’s baby book. Unfortunately, it is not supported in the NowServing Baby Book yet. At the same time, users can’t customize the design of the baby book based on their preferences. 

If you have thoughts or other narratives you want to include to make every milestone achieved more memorably, we suggest using your paper baby book for now.

Is it possible to edit the doctor’s entry or if the doctor can edit my entries?

On the other hand, this question from users is important to address too, and the answer is no. Parents cannot edit the pediatrician’s entry, and vice versa. That’s the design intended to maintain accuracy and legit findings from the doctor. 

Furthermore, to prevent confusion on the entries, the design intended in all trackers include a small bubble with the doctor’s photo. Thus, allowing you to easily identify whether or not that update is from you or the doctor.

Is it possible to track growth and milestones for premature babies?

Preterm birth is possible, especially if the parents have chronic conditions, vaginal bleeding, and other factors that led to early childbirth. Unfortunately, parents cannot use the NowServing Baby Book to track growth and milestones for preterm cases.

Make sure to consult with your baby’s pediatrician on how to properly assess your child’s development.

Can a medical doctor in the barangay health center update my NowServing Baby Book?

Other cases may happen wherein a parent visits a local health center for free immunizations for their baby. In that case, you cannot let that health expert access your baby’s baby book. What you can do instead is to inform your NowServing pediatrician about this outside session. 

You can also update the baby book on your end. If the session involved vaccination, just update which type of vaccine and the date your baby received the immunization. 

Can I access the baby book offline?

While you can open the baby book on your device without an internet connection, it doesn’t mean that you can access the full health data of your baby. At the same time, you cannot edit and save a new entry on the baby book if you update anything offline. It’s best to make sure that your internet connection is stable before opening and updating the NowServing Baby Book.

What is an Annual Physical Exam?

Annual Physical Exam or APE is a screening test, not a treatment. Nobody is looking for anything in particular. Such tests are cheap, broad, and chosen because they catch common things early like anemia or parasites in the body.

It can screen conditions like the following:

• kidney problem
• urinary problem
• lung problem

That is also why it repeats every year. One CBC is a snapshot. Five years of CBCs is a trend, and the trend is usually what a doctor is reading.

What “Medical” Means When HR Says It?

Most Filipinos don’t say “annual physical examination” out loud. They say “medical,” as in “may medical kami sa office next week.”

If your HR just says “medical,” ask two things: which tests are covered, and whether you’re paying for anything. Those two answers decide whether you need to get an APE, Pre-employment Medical Clearance, or Executive check-up.

APE vs. Pre-employment Medical Exam

Both screening tests include the Basic 5, their difference is the purpose. A pre-employment medical decides whether you are fit for a particular job, and it ends in a fit-to-work classification. An annual physical exam is for someone already employed, and it ends in a set of results you keep. We covered the hiring version separately in the pre-employment medical exam guide.

APE vs. Executive Check-up

On the other hand, when it comes to these 2, their difference is the cost and types of tests involved. An executive check-up, sometimes called an ECU, is the same idea with far more tests bolted on like extended blood chemistry, a treadmill stress test, ultrasound, thyroid panels, tumor markers, and specialist consults. Some hospital versions include a room for a day or two.

The gap in price is large. Cebu Doctors’ University Hospital sells a basic check-up at PHP 7,490 and a comprehensive one at PHP 30,210 for men.

The Basic 5, and Why Nearly Every Clinic Sells The Same Thing

The Basic 5 is the de facto floor for adult screening in the Philippines. It’s not a legal standard. It’s just what almost everyone converged on.

5 Tests Everyone Agrees On

Four providers define it near-identically on their own pages: Makati Medical Center, TopHealth, Maxicare, and FriendlyCare Clinic. UniHealth Southwoods uses the same phrase for both its pre-employment and annual packages.

The five tests for APE are:

• physical exam by a doctor
• complete blood count
• routine urinalysis
• fecalysis
• chest x-ray.

The fifth slot is the only real variation with hospitals and clinics use fecalysis. Meanwhile, the Maxicare HMO allows random blood sugar or cholesterol to stand in for it instead. That’s worth knowing if you’re comparing a clinic package against an HMO card.

What Each of The Five Tests is Looking For

Complete Blood Count (CBC)

A CBC counts red cells, white cells and platelets from a small blood draw. It’s the test that usually picks up anemia, which is common here and often shows up as feeling hilo or tired for months without an obvious reason.

Routine Urinalysis

You give a urine sample in a cup. The lab reads it for infection markers, blood, protein and sugar. It’s the cheapest early look at your kidneys and urinary tract.

Fecalysis

A stool sample, checked for parasites and blood. This is the test people skip and the one clinics chase them for, because most Basic 5 packages won’t release results without it.

Chest X-ray

A front-view image of your lungs and heart shadow. In the Philippines this test carries extra weight because of tuberculosis screening, which is also why an abnormal reading makes people panic more than it should.

Physical Exam

A doctor takes your history, your blood pressure, height and weight, listens to your chest, and checks the obvious. It’s the part where you should mention anything you’ve been ignoring, whether you have a ‘sakit ng ulo’ that keeps coming back or ‘masakit ang tiyan’ after meals.

Other Tests Recommended by Doctors Aside from The Basic 5

Blood Chemistry

The most common test upgrade. This is where cholesterol and blood sugar enter the picture, which for most working adults is the reason to upgrade at all.

Makati Medical Center‘s “Chem M” panel runs 18 components, including fasting blood sugar, the full lipid profile, creatinine, uric acid, SGOT and SGPT. Their “Chem 23” adds six more.

ECG and Other Heart Tests

A 12-lead ECG is the usual next step, and it’s cheap. Above that sit the treadmill stress test and the 2D echo with Doppler, which are executive-tier items in almost every package.

Kaiser Medical Center sells its Basic 5 at PHP 850 and Basic 5 plus ECG at PHP 1,300.

Ultrasound

Whole abdomen ultrasound appears in nearly every comprehensive package. It’s how the liver, gallbladder, kidneys and pancreas get looked at.

Age and Sex Specific Additions

Executive packages branch here. Male versions add PSA, which is also sold as a standalone test and is the one most men ask about after 50. Meanwhile, female versions add Pap smear, breast ultrasound or mammogram, and sometimes a transvaginal ultrasound. Geriatric packages add bone densitometry, audiometry and cognitive assessment.

How Much Does An Annual Physical Exam Cost in The Philippines?

Every price below comes from the provider’s own published page or brochure, checked on 10 August 2026.

Entry Tier: PHP 530 to PHP 1300

TopHealth publishes its Basic 5 APE as “start at only PHP 530,” which is a starts-at figure and not a fixed price. Kaiser Medical Center’s PHP 850 is fixed and covers a consultation plus the five core tests across six branches. It can move up to PHP 1300 due to added ECG test.

That PHP 850 is the cheapest fixed, itemized, publicly published APE price we could find anywhere. 

Mid Tier: PHP 1,600 – PHP 9,999

This is where government specialty hospitals and private hospital packages sit, and where the spread gets wide for reasons that have little to do with the test list.

Notice the PHP 850 and the PHP 4,200. Same five tests, roughly five times the price. You’re paying for the facility, not the information.

Buying the Tests One by One Usually Costs More

Kaiser publishes its individual rates, so you can check the arithmetic. Urinalysis PHP 130, fecalysis PHP 120, CBC with platelet PHP 320, chest x-ray PHP 350.

The bundle wins, and it includes the doctor. That pattern held at every facility that published both.

Executive Check-ups, and why the top of the range isn’t a normal price

The like-for-like spread

Compare executive packages against each other and the honest range is roughly P6,000, at The Hospital at Maayo in Mandaue, to PHP 42,500, which is SLU Sacred Heart’s HMO rate for the male package in Baguio.

One oddity worth flagging: SLU Sacred Heart publishes an HMO rate that is higher than its cash rate on every package. Paying with your HMO card is not automatically the cheaper route.

The very top of the list

PHP 81,800 buys endoscopy and colonoscopy with sedation, specialist fees, and a hospital stay. That’s a screening package with real procedures inside it. Nobody should read it as what a check-up costs.

Why Two Price Lists from the Same Hospital Can Disagree

Makati Medical Center currently has two live price lists on its own domain. The web page says “Effective March 1, 2026.” A PDF still sitting in the site’s uploads folder says “Effective January 1, 2024,” and the numbers differ: Basic 5 at PHP 4,000 versus PHP 4,200, Executive at PHP 19,900 versus PHP 20,900.

NOTE: We used the 2026 figures throughout. If you find a cheaper number in a PDF, check the effectivity date before getting excited.

When Only An Aggregator has a Number

Several well-known providers publish no prices at all. St. Luke’s lists five package names and zero prices. The Medical City’s pages wouldn’t load. Hi-Precision publishes full test compositions for every package and no price anywhere.

Third-party sites do quote figures for these:

• Hi-Precision at PHP 1,100, PHP 2,150 and PHP 3,150
• St. Luke’s wellness range of PHP 25,000 to PHP 120,000.

Neither provider publishes those numbers themselves, so treat them as hearsay and call the hospital.

Can PhilHealth or Your HMO Cover the Cost?

PhilHealth Konsulta

Konsulta covers 15 laboratory and diagnostic items, including CBC, lipid profile, FBS, HbA1c, creatinine, chest x-ray, ECG, urinalysis, Pap smear and fecalysis. That list looks a lot like an APE, which is why people assume it’s free.

Annex B of Circular 2024-0013 words it carefully: those tests are covered “as needed as part of case management” by the primary care team you registered with. It’s a capitated primary care package, not a voucher for a yearly check-up. We found no PhilHealth page saying a standalone APE or executive check-up is reimbursable.

HMO Cards

Only one HMO we checked publishes an itemised APE with a price. Maxicare’s PRIMA Access prepaid card is P9,999 a year and includes an APE Basic 5, availed only at Maxicare Clinics, plus unlimited consults and 24 Set A lab tests.

We could not source published APE inclusions from Intellicare, PhilCare, Medicard or Cocolife. If you hold one of those, your coverage exists in your plan document, not on a public page.

Who Pays When Your Employer Requires APE

DOLE’s workplace safety rules say periodic medical exams are free of charge to workers. For how that applies to your company, ask your HR team or the DOLE regional office covering your workplace.

When Your Company Arranges APE for Everyone

If HR runs the exam for the whole office, the mechanics change. They pick a provider, lock a date, and either the clinic sends a mobile team onsite or everybody troops to a branch. TopHealth, for one, runs its own mobile clinic for exactly this.

That’s the world of company-arranged annual physical exams, and it’s why two people with the same job title can end up with very different test lists. We wrote up how companies actually schedule these in the employer’s guide to APEs.

How to Prepare for an APE?

Fasting

If your package includes fasting blood sugar or a lipid profile, you’ll be asked to fast overnight. Water is usually fine. None of the package pages we checked publishes the exact number of hours, so confirm with your clinic when you book.

Stool Sample

Collect it at home in the container the clinic gives you, or ask whether they’ll accept one collected on site. This is the single most common reason a package sits unreleased for a week.

What to Bring

A valid ID, your company’s letter of authorization if there is one, and any previous results you still have. Old results are more useful than people realize, because the comparison is the point.

If You’re Pregnant, or Having Your Period

Tell the technologist before the chest x-ray if you’re pregnant or think you might be. And many labs prefer to reschedule a urinalysis if you’re having your period, since blood in the sample muddies the reading.

What the Day Looks Like, and Does it Hurt

The honest answer to “masakit ba?” is: briefly, and only the blood draw. One needle, a few seconds, a cotton ball taped to your arm. The chest x-ray, urinalysis and fecalysis involve no pain at all.

The part people underestimate is waiting. Expect a longer queue at a hospital than at a small clinic, and expect the fasting packages to be scheduled early in the morning for that reason.

Come rested if you can. Puyat can nudge your blood pressure reading, and a high reading you caused yourself just means a repeat.

How to Read Your Results

The Reference Range is Doing The Work

Every lab prints a reference range beside each result and flags anything outside it. You don’t need to memorize normal values. You need to look at which lines got flagged.

A flag is a finding, not a diagnosis. Plenty of flagged results are meaningless on their own and only matter as a pattern. Make sure to consult with your doctor for an official reading of your test results.

Findings that Commonly Turn Up

• Low hemoglobin on a CBC
• Infection markers on urinalysis
• Parasites on fecalysis
• Elevated fasting blood sugar or cholesterol on a chemistry panel
• Something noted on the chest x-ray.

Any of those is worth bringing to a doctor rather than searching online at 1am. Internal Medicine is usually the right first stop, and they’ll refer onward if the finding needs a specialist.

The Fit-to-Work Classes

If the exam produces a classification, it comes from OSHS Rule 1967.01(3), which rates people Class A through D.

• Class A is fit for any work
• Class B covers correctible defects like refraction errors or dental caries
• Class C is employable with limited duty and follow-up, and it expressly includes conditions like hypertension and heart disease
• Class D is unfit for any employment

The reassuring reading of that scale: only active PTB sits in Class D, so an abnormal chest x-ray is a finding to follow up, not a verdict on your job.

What an Annual Physical Exam Won’t Catch

The Basic 5 doesn’t screen for cancer. It doesn’t include a diabetes test unless blood chemistry is added. It doesn’t look at your heart beyond a stethoscope unless there’s an ECG, and it doesn’t image your abdomen at all.

So a clean APE means the tests that were run came back unremarkable. It doesn’t mean nothing is wrong, and it shouldn’t stop you from seeing a doctor about a symptom that’s been bothering you. If something has been bothering you for weeks, book a consult about that specific thing rather than waiting for next year’s package to find it.

Frequently Asked Questions

Do I need to fast before an annual physical exam?

If your package includes fasting blood sugar or a lipid profile, yes, most labs ask for an overnight fast, and water is usually fine. A plain Basic 5 has no blood chemistry in it, so fasting is often not required. None of the package pages we checked publishes the exact number of hours, so confirm with your clinic when you book.

Does PhilHealth cover an annual physical exam?

Not as a standalone benefit. PhilHealth Konsulta covers 15 laboratory and diagnostic tests, but Annex B of Circular 2024-0013 says they are covered as needed as part of case management by the primary care team you registered with, which is a clinical decision rather than a yearly entitlement. We found no PhilHealth page stating that a standalone annual physical exam or executive check-up is reimbursable.

Is an annual physical exam the same as a pre-employment medical exam?

The test list is often identical, since both usually run on the Basic 5. The purpose is different. A pre-employment medical decides whether you are fit for a specific job and ends in a fit-to-work classification. An annual physical exam is screening for someone already employed, and it repeats every year so changes over time become visible.

Does my employer have to pay for my annual physical exam?

Under the DOLE Occupational Safety and Health Standards, Rule 1967, workers must undergo physical examination periodically at intervals of no longer than one year, and those examinations are to be rendered free of charge to the workers. There are carve-outs: land, sea and air transportation establishments and mining activities sit outside those rules, and the pre-employment paragraph of the same rule contemplates an applicant paying for the chest x-ray in some cases.

Can I get an annual physical exam on my own, without a company?

Yes. Kaiser Medical Center sells its Basic 5 at PHP 850 across six branches, Asian Hospital sells wellness cards from PHP 1,800, and AIDE runs a home-service package in NCR at PHP 5,000 to PHP 5,700 for 14 tests. You do not need an employer or an HMO referral to buy any of them.

What happens if something abnormal shows up?

The lab prints a reference range beside every result and flags anything outside it, and a flag is a finding rather than a diagnosis. Bring the flagged results to a doctor, usually Internal Medicine first, and let them decide whether it needs repeating, monitoring or a referral. An abnormal chest x-ray in particular is worth following up, and under the OSHS fitness classification only active PTB falls in the unfit-for-any-employment class.

What is a Company Annual Physical Exam Program?

The company version of an APE is the same clinical event as an individual one. What changes is that you’re buying it in bulk, on a schedule, for people who have opinions about it.

The Basic 5: What Most Programs Are Built On

Four providers define the Basic 5 near-identically on their own pages: Makati Medical Center, TopHealth, Maxicare and FriendlyCare. It’s a physical exam by a doctor, a CBC, a routine urinalysis, a fecalysis, and a chest x-ray.

The one real variation is the fifth slot: clinic and hospital versions use fecalysis, while the Maxicare version allows a random blood sugar or cholesterol test instead. If you’re comparing a clinic quote against an HMO card, that’s the line to check.

What HR Owns, and What the Doctor Owns

HR owns scope, schedule, budget and billing. That’s the whole list. What the results mean, whether someone is fit to work, and what happens next are decisions that belong to the examining doctor, typically an occupational medicine specialist. 

This split matters more than it sounds, and the section on what HR is allowed to see is where it becomes concrete.

Step 1: Decide Who Is Covered

Scope decides your budget, your schedule and half your headaches. Settle it before you call a single provider.

Regular Staff, Probationary Staff, Contractors

Full-time employees are the easy part. The awkward ones are probationary staff, project-based hires, and contractors billed through an agency.

Most companies include probationary staff. Contractors are usually covered by whoever employs them, which is the agency, not you. Confirm that in the service agreement instead of assuming, because a contractor who was never examined by anyone is an easy gap to miss.

The Core Exam vs the Extras You Choose to Give

The core exam and the extras are two different budget lines, and it helps to keep them separate on paper. The Basic 5 is the core. A treadmill stress test, a thyroid panel or an ultrasound are things the company chooses to give, usually to senior staff, and they’re the first things to cut when the number comes back too high.

Tiering by Role

Splitting packages by role this way is normal. A common shape is Basic 5 for everyone, Basic 5 plus a blood chemistry panel and ECG at manager level, and an executive check-up package for the leadership team.

Two cautions. Age is a more defensible tier than seniority if anyone asks why, and a tier that reads as a status symbol tends to get talked about in the pantry.

Check What Applies to Your Company

Workplace medical exams fall under DOLE’s occupational safety rules. For what they require of your company, ask your HR or legal team, or the DOLE regional office covering your workplace.

Step 2: Pick a Delivery Model

There are three ways to actually get 200 people examined, and they fail in different ways.

Onsite Mobile Clinic

A provider brings staff and equipment to your office, usually including a mobile x-ray unit, and runs the whole batch over one to several days.

Most HR teams don’t know this exists. TopHealth runs its own mobile clinic for exactly this kind of program, and its APE packages start at PHP 530. The tradeoff is that you need parking, a room, and power, and you’re committed to whatever dates the mobile team has open.

Employees Go to a Branch on Staggered Dates

You buy a package, the provider issues a letter of authorization, and employees show up at a branch on assigned days.

This is the least demanding of your facilities and the hardest to chase. Somebody always doesn’t go. Expect to run a second round for stragglers and to build that into your timeline from the start.

Reimbursement

Employees arrange their own exam at any accredited facility and claim the cost back against a cap.

It’s the lightest to administer at the front and the heaviest at the back, because you’ll get twelve different test lists and a shoebox of receipts. This is also the only model where a comparable result across the workforce is basically impossible.

Here’s how the three models compare on the factors that actually decide which one fits your workforce:

Step 3: Choose a Provider and Get a Quote

Assume you’ll have to ask for the price. The first quote you get typically isn’t itemised either.

The Seven Things to Ask for in Writing

Before you commit to a provider, get these seven items confirmed in writing:

• Itemised test list: Ask for the actual tests included, not just the package name.
• Effectivity date: Ask for the effectivity date on the price list, since a provider can have two versions live at once.
• Reporting turnaround: Ask for the turnaround in working days.
• Doctor interpretation: Ask whether a doctor reads and signs the results, or whether you’re buying lab work with no interpretation.
• Fit-to-work certificate process: Ask how it’s issued and to whom.
• Add-on pricing: Ask what an add-on test costs if an employee wants one.
• No-show policy: Ask what happens on a no-show, because that’s where the surprise line on the invoice comes from.

The one worth pushing on is whether a doctor reads the results. A package with no interpretation leaves your people holding numbers they can’t act on, and it’s the cheapest way for a quote to look competitive.

Why the Effectivity Date Matters

That’s a small gap on one head and a real one on 500. Get the date on the document you’re quoting from.

Many Big Names Publish No Prices at All

St. Luke’s lists five package names and no prices. Hi-Precision publishes the full test composition of every package and no price anywhere. The Medical City’s wellness pages returned errors when we checked them in August 2026.

So for a large part of the market, a quote only exists after an inquiry, and third-party price lists for those providers are hearsay. Budget the calendar time for that, roughly a week of back and forth before you have three comparable numbers.

Going Through a Booking Platform Instead

Step 4: Cost the Program per Head

As of 10 August 2026, every figure below is a published retail price from the provider’s own page or brochure. Corporate rates are negotiated per head and a real quote will differ, usually downward with volume. We’re not publishing a corporate discount rate because we don’t have a sourced one.

Published Per-Head Prices to Build a First Budget From

Published retail runs from PHP 530 for a basic package to PHP 9,999 for a full-year HMO-based plan, and what’s included varies as much as the price:

Note the PHP 850 and the PHP 4,200. Same five tests, roughly five times the price, and the difference is the facility rather than the information.

What a 50-Person and a 200-Person Program Costs

This is published retail multiplied by headcount. Nothing else is in it, no negotiated rate, no onsite fee, no add-ons.

A useful way to present this to finance is as a range with the tier named, rather than a single number. The spread between the bottom and top row is the entire conversation.

Where the HMO Sits

An HMO card and an APE program overlap less than people expect. Only Maxicare publishes an itemised APE with a price, PRIMA Access at PHP 9,999 a year, and the APE in it can only be availed at Maxicare Clinics.

One oddity worth knowing before you assume the card is cheaper. SLU Sacred Heart publishes an HMO rate higher than its cash rate on every package.

Step 5: Schedule a Whole Workforce

This is the part that turns a good quote into a bad week.

Fasting Packages Have to Run Early

Anything with fasting blood sugar or a lipid profile needs an overnight fast, so those slots are early morning. That caps how many people you can process before the fasting window closes.

A plain Basic 5 has no blood chemistry in it, so fasting is often not required at all. Confirm which one you bought before you tell 200 people not to eat.

The Stool Sample Is What Delays Your Batch

Fecalysis containers have to go out the day before, and the sample gets collected at home. This single item is the most common reason a batch of results sits unreleased.

Send the containers with written instructions. A verbal briefing at the assembly won’t survive the trip home. Then plan a catch-up window, because a chunk of people will forget on the first pass.

Stagger by Department, Not Alphabetically

Split by department or team so no function is fully offline at once, and keep the customer-facing teams off the same slot. Give each group a two-hour window rather than a day.

Leave the last slot of each day empty on purpose. It absorbs the late arrivals, and without it they roll into tomorrow’s fasting group.

Companies That Want the Whole Thing Handled

Step 6: Handle the Results

Turnaround time, who receives what, and what happens after an abnormal finding all get decided in this step.

Turnaround

Routine results are typically ready in a few working days, with x-ray reading and any specialist review taking longer. Ask for the turnaround in working days, and ask what happens to it when a sample has to be repeated.

Who Receives What

The short answer is that employees receive their own results and the company receives fit-to-work status, which is the same shape as the medical certificates companies already accept for sick leave. Those reasons aren’t optional, and the next section spells them out.

The Abnormal-Result Path

The examining doctor flags what needs follow-up and tells the employee. HR’s job at that point is administrative: make the follow-up possible, protect the confidentiality of the finding, and don’t act on a diagnosis you were never supposed to have.

What HR Is Allowed to See

This is the part of a program most companies get wrong by default, usually because someone asked for a spreadsheet of results and nobody pushed back.

Health results are sensitive personal information, so keep HR’s access narrow. For the privacy rules that apply to your company, check with your data protection officer or the National Privacy Commission.

The Practical Design Rule

Design the program so the company receives fit-to-work status and the employee receives the full results. It’s the easier shape to run, because it removes any reason for HR to be holding clinical data at all.

The aggregate row is the one worth negotiating for. A de-identified summary, such as how many people were referred for follow-up, is useful for planning next year’s program and carries none of the individual data.

When a Result Comes Back Abnormal

Mostly, HR does nothing. The doctor decides, not the company.

The Fit-to-Work Classes

OSHS Rule 1967.01(3) sets four classes, and clinics didn’t invent them:

• Class A: “Physically fit for any work”
• Class B: “Physically under-developed or with correctible defects, (error of refraction dental caries, defective hearing, and other similar defects) but otherwise fit to work”
• Class C: “Employable but owing to certain impairments or conditions, (heart disease, hypertension, anatomical defects) requires special placement or limited duty in a specified or selected assignment requiring follow-up treatment/periodic evaluation”
• Class D: “Unfit or unsafe for any type of employment (active PTB, advanced heart disease with threatened failure, malignant hypertension, and other similar illnesses)”

Read Class C carefully. It expressly contemplates continued employment with limited duty and follow-up, which covers most of what an APE actually turns up in a working-age population.

An Abnormal Chest X-Ray

A flagged chest x-ray is a finding, not a verdict. Only active PTB sits in Class D under that classification.

The employee needs a referral and follow-up. What they don’t need is a decision about their job made from a radiology line by someone who isn’t a doctor.

Hepatitis B, HIV and TB

Government agencies set rules on screening and hiring for these conditions, and those rules change. Don’t add Hepatitis B or HIV tests to the package without checking with DOLE or a labor lawyer first.

Step 7: Sort Out Billing

The invoice at the end of a program rarely matches the quote exactly, and the gap usually comes down to three questions.

Per Head Versus Package

Most providers quote per head against a named package, and the invoice is headcount multiplied by the rate. Some quote a program price for a fixed batch size, which is worth checking, because you’ll pay for the no-shows either way.

Confirm the no-show treatment in writing before the first batch. It’s the single most common billing dispute in these programs.

Where the HMO Sits

If your HMO card includes an APE, you’re deciding between using the card and buying a separate program. Card-based exams usually have to be availed at the HMO’s own clinics, which removes the onsite option and puts the scheduling back on the individual employee.

Running a separate APE program alongside an HMO is normal. They’re different purchases and they don’t have to be settled at the same time of year.

What Finance Should Expect to Be Invoiced For

The package rate multiplied by heads examined, any add-on tests employees chose, and, in an onsite program, sometimes a mobilisation or minimum-batch fee.

At the top sits Makati Medical Center’s Comprehensive Abdominal A inpatient package at PHP 81,800, which includes endoscopy and colonoscopy with sedation, so it’s a screening-plus-procedure package rather than a check-up. Among plain check-ups, the highest published figure is MakatiMed’s HealthHub CVD Screening A at PHP 68,500. Every figure below was read off the provider’s own page or its own PDF on 10 August 2026.

Most price articles on this topic give you a category range and no provider name. That’s not usable. You can’t budget from “PHP 3,000 to PHP 8,000.”

So this one is organized the way you actually choose: by tier, by provider type, and by where you are. Where a provider doesn’t publish a price, we say that plainly instead of filling the gap.

What the Money Actually Buys

Package names are marketing. The test list is the product. Once you can read a test list, the price differences stop looking random.

The Basic 5, the National Floor

A physical exam, a complete blood count, routine urinalysis, fecalysis and a chest x-ray make up the Basic 5. Makati Medical Center, TopHealth and FriendlyCare all define it that way, near-identically.

The one real variation is the fifth slot: hospital and clinic versions use fecalysis, while Maxicare’s HMO version allows a random blood sugar or a cholesterol test instead. This is the tier most people mean when they say “medical” or “APE,” and it’s the tier that gets you a fit-to-work certificate.

Basic 5 Plus an ECG or a Chemistry Panel

The next step up adds a 12-lead ECG, a blood chemistry panel, or both. Kaiser sells Basic 5 plus ECG at PHP 1,300. Philippine Heart Center’s Basic Adult Package, at PHP 3,150, swaps in a fasting blood sugar and a lipid profile.

Same idea, different assembly. This is where most corporate APE programs sit.

The Comprehensive and Executive Tier

Above roughly PHP 7,000, the packages start looking alike across providers. They add:

• Extended chemistry panel: A broader blood workup than the Basic 5’s core tests.
• Cardiac workup: Usually an ECG paired with a treadmill stress test or imaging component.
• Whole abdomen ultrasound: Screens the abdominal organs as a group.
• Thyroid and metabolic markers: Tests such as TSH alongside blood sugar and lipid markers.
• Tumour markers: Blood tests used as an early screen for certain cancers.
• Sex-specific tests: A Pap smear or breast ultrasound for women, a PSA test for men.

At the top of this tier you’re also paying for consultations. SLU Sacred Heart’s executive package includes five specialist consults.

The Inpatient Tier

Inpatient packages put you in a private room for one or two days and bundle in meals, parking and professional fees, making them the most expensive tier on this list. Makati Medical Center’s Health Investment Inpatient A is PHP 26,600. Philippine Heart Center’s 24-hour Cardio-Pulmonary Exam is PHP 36,500 for men.

You’re buying convenience and pacing here as much as tests. Everything happens in one stay instead of three trips.

The Cheapest Legal Check-Up in the Philippines

People usually ask this question when a job or a permit needs a result by a date. Here’s the honest answer.

The Clinic Chain Floor

Kaiser Medical Center’s Package A at PHP 850 is the lowest fixed published price we found anywhere. TopHealth publishes PHP 530, but it’s written as a starting price, so treat it as a floor you may not actually get.

SLU Sacred Heart in Baguio prices the same five items at PHP 1,199 under a pre-employment label. A private tertiary hospital, for close to clinic money.

The Government Hospital Route

Philippine Heart Center publishes a Basic Pre-Employment Package at PHP 1,500 and a Mini-Adult Package at PHP 1,600. Both are real published prices at a government specialty hospital in Quezon City.

Bulacan Medical Center, a government provincial tertiary hospital in Malolos, publishes individual test rates rather than a package:

• CBC: PHP 150
• Urinalysis: PHP 170
• Fecalysis: PHP 50
• Chest PA: PHP 150

The LGU Health Certificate Route

City health offices run the cheapest lab rates in the country. Quezon City charges PHP 40 for a stool exam and PHP 97 for sputum, while Baguio charges PHP 100 for stool and PHP 100 for a chest x-ray. Batangas City charges PHP 140 for a chest PA.

Take note: That route buys a health certificate for work or a sanitary permit, not an annual physical exam. Requirements differ by city, and even fecalysis validity varies, one month in Malabon and Baguio, three months in Quezon City, six months in Muntinlupa. Buying the Tests One by One Costs More Than the Bundle

Everyone assumes this. Almost nobody has done the arithmetic against a published list, because you need a provider that publishes both, and most don’t.

The Kaiser Arithmetic

As of 8 June 2026, Kaiser publishes an individual test list alongside its packages:

• Routine urinalysis: PHP 130
• Fecalysis: PHP 120
• CBC with platelet: PHP 320
• Chest x-ray: PHP 350

That’s PHP 920 for four items, against PHP 850 for Package A, which includes those four plus the consultation. So you pay PHP 70 more for the parts and you don’t get the consult. Small, but it settles the question with real numbers instead of a rule of thumb.

When Paying per Test Still Makes Sense

If your company already covered your chest x-ray last quarter, or you only need one test repeated, the à-la-carte list is the cheaper path. A repeat FBS at Kaiser is PHP 160 against PHP 850 for the whole package.

The same logic applies to the tests people buy on their own rather than in a package. We priced two of the common ones separately, the PSA test and a hormone panel, and in both cases it is worth checking whether the package you are considering already includes them before you pay twice. Buying the bundle wins when you need the full set, and it loses when you need one thing.

Metro Manila Package Prices

Metro Manila has the deepest bench of published APE prices in the country, and three hospitals cover the full spread from entry to inpatient.

Makati Medical Center

MakatiMed publishes the most complete price list of any hospital we checked, thirteen packages with itemised inclusions. It’s also the only hospital whose list spans the entire market, from a PHP 4,200 Basic 5 to PHP 81,800.

Worth knowing before you compare: the PHP 81,800 package is not a check-up in the ordinary sense. It includes an EGD and a colonoscopy under sedation with gastroenterology and anaesthesia fees.

Philippine Heart Center

PHC is a government specialty hospital, and its outpatient packages are priced accordingly. The Hypertension Package is PHP 4,500 and the Diabetes Package is PHP 3,600, both built around a condition rather than a general screen.

Its inpatient packages are not cheap. The 24-hour Cardiovascular Check-up is PHP 40,500 for men and PHP 41,200 for women, room and professional fees included.

Manila Doctors and Asian Hospital

Manila Doctors Hospital publishes four executive tiers, Diamond at PHP 30,000 down to Silver at PHP 21,850. The inclusions aren’t itemised on the page, so ask for the test list before you book.

Asian Hospital takes a different approach. Instead of executive packages, it publishes four Care on Demand wellness cards, from a PHP 1,800 Senior card to a PHP 12,000 heart screening card.

Cebu Package Prices

Outside Metro Manila, Cebu has the most complete published pricing, led by two hospitals with clear tier ladders.

Cebu Doctors’ University Hospital

CDUH publishes a full executive check-up ladder with letter codes, and it’s the clearest tier structure we found outside Metro Manila. A1 at PHP 7,490 is the entry rung, and it’s better value than it first looks: 19 lab tests plus an ECG and a chest x-ray.

Search snippets describe A1 as a six-test package, but the hospital’s own page lists 19, so trust the hospital page. The ladder also prices men and women differently, and not always in the same direction. C1 for men is PHP 30,210 while C2 for women is PHP 25,530, but at the A tier the female package is the cheaper one by PHP 188.

The Hospital at Maayo

Maayo in Mandaue publishes two tiers, PHP 6,000 and PHP 18,800. The PHP 6,000 Basic Executive Package includes a full chemistry panel, a 12-lead ECG, a chest PA, a post-consultation and a meal.

That’s the cheapest published package we found anywhere that includes both a chemistry panel and a consult.

Baguio, and the HMO Rate That Costs More Than Cash

SLU Sacred Heart in Baguio publishes both its regular and HMO rates side by side, which surfaces a finding worth knowing before you book.

What the SLU Sacred Heart Executive Check-Up Includes

SLU Sacred Heart Medical Center publishes its packages in its own brochure. The executive check-up is a two-day stay that includes:

• Chemistry panel
• HbA1c
• Lipids
• 2D echo
• Chest x-ray
• Whole abdomen ultrasound
• Thyroid tests
• Tumour markers
• Five specialist consults

Its GI Wellness package, an outpatient tier, is PHP 15,000 at the regular rate.

The HMO Rate Runs Higher, on Both Executive Packages

This is the finding most people won’t expect: on both executive packages where SLU prints two rates, the HMO rate is the higher one. The male package is PHP 40,500 regular and PHP 42,500 HMO, and the female package is PHP 38,000 regular and PHP 41,000 HMO.

That’s PHP 2,000 and PHP 3,000 more for arriving with a card. We can’t speak to the reason, and it may reflect billing and collection terms rather than the care itself, but the practical point stands either way: don’t assume your HMO rate is the lower one, so ask for both before you book.

Home Service and Onsite APE

Not every APE happens at a facility. Two providers publish prices for check-ups that come to you instead.

AIDE

Home service phlebotomy across NCR is AIDE’s model, and it publishes ranges rather than fixed prices. The Basic Health Package with 14 tests is PHP 5,000 to PHP 5,700, while the General Health Package with 24 tests runs PHP 9,000 to PHP 9,700.

Note what’s missing from both: there’s no chest x-ray, because a home visit can’t do one. So a home package isn’t a swap for a Basic 5, it’s a different product.

TopHealth’s Mobile Clinic

The lowest APE figure in this article, “start at only P530,” comes from TopHealth, which runs its own mobile clinic for onsite company APEs. Add-ons are quoted separately, so the PHP 530 is a floor.

If you’re arranging check-ups for a whole team rather than for yourself, pricing works differently and usually gets negotiated per head.

The One HMO Package With a Published APE Price

Most HMOs don’t publish what an APE costs under their plans, but one prepaid card does.

Maxicare PRIMA Access

A prepaid card at PHP 9,999 a year, PRIMA Access, includes an annual physical exam described as a Basic 5, plus unlimited consults, 24 Set A lab tests and two chest x-rays. The APE can only be taken at Maxicare Clinics.

It’s the only HMO product we could find that publishes an itemised APE with a price. We could not source APE coverage details from Intellicare, PhilCare, Medicard or Cocolife, so we’re not going to guess at it.

PhilHealth Konsulta Is Not an APE Benefit

This one costs people real money, because they assume they’re covered and skip budgeting for it.

The 15 Tests, and the Words Around Them

Annex B of PhilHealth Circular 2024-0013 lists 15 covered laboratory and diagnostic tests:

• CBC with platelet count
• Lipid profile
• Fasting blood sugar (FBS)
• Oral glucose tolerance test (OGTT)
• HbA1c
• Creatinine
• Chest x-ray
• Sputum microscopy
• Electrocardiogram (ECG)
• Mammogram
• Ultrasound, limited to the upper abdomen, pelvic area and breast
• Urinalysis
• Pap smear
• Fecalysis
• Fecal occult blood test

The wording around that list is what matters. They’re covered “as needed as part of case management” by your PhilHealth Konsulta primary care team, at the provider you registered with.

That’s clinical discretion, not a voucher. Your Konsulta doctor orders what your situation calls for. You can’t walk in and claim the list as an annual physical.

PhilHealth Pays the Provider PHP 1,700 a Year per Member

Annex I sets the maximum per capita amount at PHP 1,700 per member per year for government and private Konsulta providers alike, with payment released to providers in two tranches. That figure is paid to the provider, not to you, and it covers primary care for the year as a capitated primary care package.

The Same Hospital Can Publish Two Prices at the Same Time

Price lists don’t always get taken down when they’re replaced, and MakatiMed is a clear example of what that looks like.

MakatiMed’s January 2024 PDF Against Its March 2026 Page

While checking prices we found two live MakatiMed price lists on the hospital’s own domain. The HTML page is marked “Effective March 1, 2026.” A PDF still sitting under a 2024 uploads folder is marked “Effective January 1, 2024.”

They disagree because they’re two years apart. Nothing improper is happening here; an old file simply stayed online, which happens on nearly every large site. The gap is about 5% across two years, useful to know if you’re budgeting from a figure someone forwarded you.

Ask for the Effectivity Date

The practical habit: whatever price list you’re handed, sa PDF man o sa website, look for the effectivity date before you plan around it. If there isn’t one, ask.

The Big Names That Publish No Price at All

Not every hospital plays by the same rules. Several well-known names simply don’t list a number.

What They Publish Instead

Several of the hospitals and labs people ask about most don’t put a number on a page. This is a fact about the market rather than a criticism, and it’s useful to know before you spend an afternoon looking.

• St. Luke’s Medical Center: Lists five wellness package names with no prices attached.
• The Medical City: Its wellness pages returned 404 and 500 errors when we fetched them on 10 August 2026.
• Hi-Precision Diagnostics: Publishes the full test composition of every package, Routine through Executive Health Care, with no prices anywhere.
• Healthway QualiMed: Its pages blocked our automated fetch, an ordinary anti-bot setting.
• Cardinal Santos: Returned an empty archive page.

For all of them the price comes from an inquiry, so budget a phone call. Ask for the effectivity date on whatever they send. If you would rather start from facilities that publish what they do, our hospital directory is a shorter route.

The Figures You Will See Quoted That No Provider Published

Search results surface plenty of check-up prices that never actually came from the provider. Here’s where those numbers break down.

The “Up to PHP 120,000” St. Luke’s Number

You’ll see a PHP 25,000 to PHP 120,000 range for St. Luke’s executive check-ups repeated across the web.

Same for the rest of the table below: these are aggregator claims, and in each case the provider does not publish the number itself. Three are worth naming specifically:

• Asian Hospital and Medical Center: The aggregator’s Starter through Advance 2 tiers contradict both its own separate article, which says Asian Hospital executive check-up prices require direct inquiry, and the hospital’s own site, which publishes only the four wellness cards. We’re leaving those numbers out of the table entirely rather than repeating them.
• Hi-Precision Diagnostics: Its figures exist nowhere on Hi-Precision’s own site.
• Cardinal Santos Medical Center: Its “from P23,000” figure is unsourced against the hospital’s own pages.

Which Tier Is Actually for You

What you need tested is your doctor’s call, not ours. What follows is only about which price tier each package is built for.

First APE, Healthy, Under 35

The Basic 5 tier is what these packages are designed for, and it’s what most employers ask for. Kaiser at PHP 850, TopHealth from PHP 530, SLU Sacred Heart at PHP 1,199.

Paying PHP 20,000 for a first check-up in your twenties buys you tests aimed at risks the package assumes you have.

35 and Up, or Already Watching Blood Pressure and Sugar

This is where the ECG and chemistry tiers are aimed. Kaiser’s Basic 5 plus ECG at PHP 1,300, PHC’s Basic Adult at PHP 3,150, Maayo’s Basic Executive at PHP 6,000, CDUH’s A1 at PHP 7,490.

PHC’s condition-specific packages sit here too, Hypertension at PHP 4,500 and Diabetes at PHP 3,600, and they’re priced below most general screens.

Family History, or the Company Is Paying
